Generally, this means a minimum of six years from the date of the last treatment, or, for records of a minor patient, until three years after the patient turns 18 (or six years from the last treatment date, whichever is later), consistent with New York recordkeeping rules for dental and medical providers.

To exercise any of these rights, contact us using the information in Section 10. We may need to verify your identity before fulfilling a request, and certain records (such as parts of your clinical file) may be retained as required by law even after a deletion request.

8. HIPAA Notice of Privacy Practices

 
 

As a healthcare provider, the Practice also maintains a separate HIPAA Notice of Privacy Practices that describes in detail how your protected health information (PHI) may be used and disclosed, and your rights regarding that information. That notice is available at our office, upon request, and (where posted) on our website, and it governs your PHI to the extent it conflicts with this general website Privacy Policy.

9. Children's Information

 

Our website is intended for use by adults seeking information about dental services. We do not knowingly collect personal information directly from children through the website. Information about minor patients is collected only in the context of providing dental care, with parental or guardian involvement, as part of our clinical records.
